As technologies innovate and more complex instruments need to be reprocessed, it is crucial to focus on optimizing workflows. Due to the complex design of da Vinci Xi 8mm instruments, some precleaning manual steps are required for successful reprocessing. Reprocessing of da Vinci Xi 8mm instruments requires additional time compared to other surgical instruments. Below is an example of the Intuitive IFU, which shows all required manual pre-cleaning steps described in detail.<\/span><\/p>\n<p><img decoding=\"async\" class=\"aligncenter wp-image-275758 size-full\" src=\"https:\/\/www.matachana.com\/wp-content\/uploads\/2025\/05\/reprocesamiento-davinci-1.png\" alt=\"\" width=\"472\" height=\"379\" srcset=\"https:\/\/www.matachana.com\/wp-content\/uploads\/2025\/05\/reprocesamiento-davinci-1.png 472w, https:\/\/www.matachana.com\/wp-content\/uploads\/2025\/05\/reprocesamiento-davinci-1-300x241.png 300w\" sizes=\"(max-width: 472px) 100vw, 472px\" \/><\/p>\n<p><span class=\"fontstyle0\">For reprocessing da Vinci Xi 8mm instruments, a custom wash cycle is required, validated and provided by the washer disinfector manufacturers. Such a cycle is currently longer in duration than an MIS cycle; however, there are developments in optimizing such cycles as well.<\/span><\/p>\n<p><span class=\"fontstyle0\">Common Washer disinfector current profile:<\/span><\/p>\n<ol>\n<li><span class=\"fontstyle0\">Pre-wash<\/span><\/li>\n<li><span class=\"fontstyle0\">1st Phase of washing<\/span><\/li>\n<li><span class=\"fontstyle0\">2nd Phase of washing<\/span><\/li>\n<li><span class=\"fontstyle0\">1st Rinse<\/span><\/li>\n<li><span class=\"fontstyle0\">2nd Rinse<\/span><\/li>\n<li><span class=\"fontstyle0\">Thermal disinfection<\/span><\/li>\n<li><span class=\"fontstyle0\">Drying<\/span><\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<h2><strong><span class=\"fontstyle0\">SCOPE OF INNOVATION<\/span> <\/strong><\/h2>\n<p><span class=\"fontstyle0\">Thanks to MATACHANA &#8216;s experience as a leading manufacturer in the medical device reprocessing sector and faced with the imperative need to validate every stage of reprocessing on our medical devices, we were confident that it was possible to completely automate the reprocessing of the EndoWrist Xi 8mm Instruments. Therefore, we took on the challenge of optimizing the process, considering the following premises:<\/span><\/p>\n<ul>\n<li><span class=\"fontstyle0\">Due to its morphology and internal architecture, it is a complex that requires specific manual precleaning treatment.<\/span><\/li>\n<li><span class=\"fontstyle0\">They are reusable instruments with limited uses that require careful and gentle treatment to avoid\u00a0<\/span><span class=\"fontstyle0\">any damage or reduction in their intended lifespan.<\/span><\/li>\n<li><span class=\"fontstyle0\">The process was to improve ergonomics and reduce staff time.<\/span><\/li>\n<li><span class=\"fontstyle0\">Its design reduced total reprocessing time, increasing material turnover and availability.<\/span><\/li>\n<li><span class=\"fontstyle0\">The consumption of energy, water, and detergents needed in the overall process was to be\u00a0<\/span><span class=\"fontstyle0\">reduced.\u00a0<\/span><\/li>\n<li><span class=\"fontstyle0\">The material was to be unloaded in the Inspection and Preparation area, after the corresponding\u00a0<\/span><span class=\"fontstyle0\">cleaning, thermal disinfection, and drying stage, avoiding any possibility of recontamination caused\u00a0<\/span><span class=\"fontstyle0\">by additional handling in the dirty area before its transfer to the clean area.<\/span><\/li>\n<li><span class=\"fontstyle0\">The cleaning process had to be validated.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h2><strong><span class=\"fontstyle0\">TYPE TESTS<\/span> <\/strong><\/h2>\n<p><span class=\"fontstyle0\">To validate the effectiveness of instrument cleaning, different procedures, processes, and tests were defined, considering the most demanding conditions:<\/span><\/p>\n<h3><span class=\"fontstyle0\" style=\"color: #808080;\">INSTRUMENTS CONSIDERED THE WORST CASE <\/span><\/h3>\n<p><span class=\"fontstyle0\">Specifically, the cauterization instruments &#8220;Maryland Bipolar Forceps, ref. 470172&#8221; and &#8220;Monopolar Curved Scissors, ref. 470179&#8221; were identified as the most challenging instruments for cleaning and thus representative for the Xi 8mm Instruments family. During the cauterization process, residues of burned tissue typically remain at the instrument\u2019s distal tip.<\/span><\/p>\n<p><img decoding=\"async\" class=\"aligncenter wp-image-275825 size-full\" src=\"https:\/\/www.matachana.com\/wp-content\/uploads\/2025\/05\/reprocesamiento-davinci-13.png\" alt=\"\" width=\"257\" height=\"123\" \/><\/p>\n<p>&nbsp;<\/p>\n<h3><span class=\"fontstyle0\" style=\"color: #808080;\">SOILING<\/span><\/h3>\n<ul>\n<li><span class=\"fontstyle0\">Coagulated sheep blood was used as soil according to the recommendations of ISO 15883-5 and\u00a0<\/span><span class=\"fontstyle0\">AAMI TIR 30:2011 (later replaced by ANSI\/AAMI ST98:2022) Standards. A volume of 70 \u00b5l was\u00a0<\/span><span class=\"fontstyle0\">defined for soiling the interior channel, injected and distributed throughout the lumen, and soiling the\u00a0<\/span><span class=\"fontstyle0\">distal tip by immersion in the same solution.<\/span><\/li>\n<li><span class=\"fontstyle0\">It was left to dry for 60-65 minutes at room temperature to ensure the worst-case scenario and\u00a0<\/span><span class=\"fontstyle0\">represent the greatest challenge to the cleaning process.<\/span><\/li>\n<li><span class=\"fontstyle0\">As the inner instrument channel is not visually accessible, a first test was conducted using the Radionuclide Method (RNM). By measuring the radiation, it was verified that the distribution of dirt on the instrument was optimal by the soiling method used.<\/span><\/li>\n<\/ul>\n<p><img decoding=\"async\" class=\"aligncenter wp-image-275805 size-full\" src=\"https:\/\/www.matachana.com\/wp-content\/uploads\/2025\/05\/reprocesamiento-davinci-9.png\" alt=\"\" width=\"644\" height=\"180\" srcset=\"https:\/\/www.matachana.com\/wp-content\/uploads\/2025\/05\/reprocesamiento-davinci-9.png 644w, https:\/\/www.matachana.com\/wp-content\/uploads\/2025\/05\/reprocesamiento-davinci-9-480x134.png 480w\" sizes=\"(min-width: 0px) and (max-width: 480px) 480px, (min-width: 481px) 644px, 100vw\" \/><\/p>\n<p><span class=\"fontstyle0\">Although this test is not according any standard, it shows clearly the real distribution of the soil.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><span class=\"fontstyle0\" style=\"color: #808080;\">REQUIREMENTS FOR AUTOMATED PRE-CLEANING <\/span><\/h3>\n<ul>\n<li><span class=\"fontstyle0\">The process reduces user manipulation to placing and removing instruments from the pre-washing\u00a0<\/span><span class=\"fontstyle0\">unit.<\/span><\/li>\n<li><span class=\"fontstyle0\">Subsequently, the EndoWrist instruments are subjected to an ultrasonic irrigation process with distal\u00a0<\/span><span class=\"fontstyle0\">tip movement for 10 minutes. This process is performed on the MATACHANA MAT UIRI unit<\/span > which includes a Trison 4000 system manufactured by Bandelin.<\/li>\n<li><span class=\"fontstyle0\">Following this procedure, the instruments are visually inspected using a magnification glass per\u00a0<\/span><span class=\"fontstyle0\">Intuitive\u2019s RIFU, and the distal tip is brushed if necessary.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h3><span class=\"fontstyle0\" style=\"color: #808080;\">REQUIREMENTS FOR CLEANING, DISINFECTION AND AUTOMATED DRYING <\/span><\/h3>\n<ul>\n<li><span class=\"fontstyle0\">The washing, thermal disinfection, and drying process is carried out in a washer disinfector\u00a0<\/span><span class=\"fontstyle0\">according to EN ISO 15883 part 1,2. For this purpose, a MATACHANA MAT LD1000 washer\u00a0<\/span><span class=\"fontstyle0\">disinfector with standard configuration, da Vinci Xi 8mm instrument rack, and conventional MIS<\/span><span class=\"fontstyle2\">[2]\u00a0<\/span><span class=\"fontstyle0\">cycle with the following profile<\/span><span class=\"fontstyle2\">[3] <\/span><span class=\"fontstyle0\">was used:<\/span>\n<ul>\n<li><span class=\"fontstyle0\">Pre-wash<\/span><\/li>\n<li><span class=\"fontstyle0\">1 Washing phase<\/span><\/li>\n<li><span class=\"fontstyle0\">2 Rising phase<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span class=\"fontstyle0\">The same enzymatic detergent suitable for the automated washing of medical devices was\u00a0<\/span><span class=\"fontstyle0\">selected. This precluded any possibility of incompatibility between different chemicals and the need\u00a0<\/span><span class=\"fontstyle0\">for intensive manual rinsing of the instruments between the ultrasonic irrigation and the washer\u00a0<\/span><span class=\"fontstyle0\">disinfector.<\/span><\/li>\n<li><span class=\"fontstyle0\">As in any Validation procedure, several reprocessing cycles were repeated with both instruments\u00a0<\/span><span class=\"fontstyle0\">defined as the worst-case. In this way, we could confirm the consistency and repeatability of the\u00a0<\/span><span class=\"fontstyle0\">results.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h3><span class=\"fontstyle0\" style=\"color: #808080;\">EXTRACTION, RECOVERY, AND QUANTIFICATION OF SOIL <\/span><\/h3>\n<p><span class=\"fontstyle0\">The extraction, recovery, and quantification methods of residues were carried out following the procedures of an accredited and independent external laboratory. This laboratory ensured the capture, evaluation, analysis and documentation of all possible residues after the process.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><span class=\"fontstyle0\" style=\"color: #808080;\">ACCEPTANCE REQUIREMENTS <\/span><\/h3>\n<p><span class=\"fontstyle0\">Acceptance requirements were assessed according to the highest standards:<\/span><\/p>\n<table class=\"NormalTable\">\n<tbody>\n<tr>\n<td style=\"border-left: solid 1px #1b365d; In all cases, the trace levels of soiling after washing were significantly lower than the established thresholds. Most samples showed protein presence values below 50% of the strictest limits.<\/span><\/p>\n<p><span class=\"fontstyle0\">The results&#8217; analysis confirmed that the new reprocessing proposal improves washing efficiency, possibly reducing manual pre-cleaning time while maintaining cleaning efficacy compared to manual pre-cleaning steps as described by the manufacturer Intuitive.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h2><strong><span class=\"fontstyle0\">FINAL CONCLUSIONS<\/span> <\/strong><\/h2>\n<p><span class=\"fontstyle0\">MATACHANA and INTUITIVE have collaborated on the development of an innovative alternative for reprocessing EndoWrist XI 8mm Instruments.<\/span><\/p>\n<p><span class=\"fontstyle0\">The new process brings numerous advantages for both the Hospital and its users:<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><strong><span class=\"fontstyle0\">1. High performance:<\/span> <\/strong><\/p>\n<ul>\n<li><span class=\"fontstyle2\">Simultaneous reprocessing of up to 14 instruments<\/span><span class=\"fontstyle3\">[4]<\/span><span class=\"fontstyle2\">, resulting in a significant increase in <\/span><span class=\"fontstyle2\">productivity.<\/span><\/li>\n<li><span class=\"fontstyle2\">The increased rotation of instruments allows for more uses during the same workday.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>\u00a0<span class=\"fontstyle0\">2. Reduction in the total reprocessing time:<\/span> <\/strong><\/p>\n<ul>\n<li><span class=\"fontstyle2\">It reduces the time required for automatic reprocessing of instruments by more than 50% and about <\/span><span class=\"fontstyle2\">75% compared to manual reprocessing.<\/span><\/li>\n<li><span class=\"fontstyle2\">Allows reprocessing of the four instruments used in a conventional intervention in only 60 minutes <\/span><span class=\"fontstyle2\">vs. &gt;120 minutes currently.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><span class=\"fontstyle0\">Could potentially reduce the current nine steps of automatic reprocessing:<\/span><\/p>\n<p><img decoding=\"async\" class=\"wp-image-275788 size-full aligncenter\" src=\"https:\/\/www.matachana.com\/wp-content\/uploads\/2025\/05\/reprocesamiento-davinci-6.png\" alt=\"\" width=\"859\" height=\"82\" srcset=\"https:\/\/www.matachana.com\/wp-content\/uploads\/2025\/05\/reprocesamiento-davinci-6.png 859w, https:\/\/www.matachana.com\/wp-content\/uploads\/2025\/05\/reprocesamiento-davinci-6-480x46.png 480w\" sizes=\"(min-width: 0px) and (max-width: 480px) 480px, (min-width: 481px) 859px, 100vw\" \/><\/p>\n<p><span class=\"fontstyle0\">To four steps, whereas this is currently not reflected in Intuitive reprocessing instructions.
